A Silent Epidemic
Non-Alcoholic Fatty Liver Disease (NAFLD) is the most common liver condition worldwide, affecting approximately 25% of the global adult population — with even higher prevalence in India and South Asia. It is closely linked to insulin resistance, obesity, and metabolic syndrome, and is now the leading cause of liver transplantation in several countries.
From Fat to Fibrosis: The Progression
NAFLD exists on a spectrum. It begins with simple steatosis — fat accumulation in liver cells without significant inflammation — and can progress to NASH (non-alcoholic steatohepatitis), where inflammation and cell damage occur. If unchecked, NASH can advance to fibrosis, cirrhosis, and eventually liver failure or hepatocellular carcinoma.
The liver can regenerate — but only if you catch the damage before fibrosis becomes irreversible. That is why early detection matters.
How Is NAFLD Diagnosed?
Transient elastography (FibroScan) — the gold-standard non-invasive tool that measures both liver stiffness (fibrosis) and fat content (CAP score) in a single 10-minute scan
Liver enzymes (ALT, AST, GGT) — elevated enzymes are a late sign; normal enzymes do not rule out NAFLD
Ultrasound — can detect moderate-to-severe steatosis but cannot quantify fat or assess fibrosis
HOMA-IR and metabolic panel — insulin resistance is the primary driver; always assess alongside liver markers
The Reversal Path
NAFLD is reversible — especially in the steatosis and early NASH stages. The interventions that work for NAFLD are the same ones that reverse insulin resistance and visceral adiposity, which is why an integrated metabolic approach is essential.
Weight loss of 7–10% — the single most effective intervention; reduces liver fat, inflammation, and fibrosis
Time-restricted eating — reduces hepatic de novo lipogenesis by lowering insulin exposure
Reducing fructose and refined carbohydrates — fructose is metabolised almost exclusively by the liver and directly drives fat synthesis
Omega-3 fatty acids (EPA/DHA) — reduce hepatic inflammation and improve lipid profiles
